The Transitional Age Youth Health and Wellness Center (TAY Center) is a welcoming place, open 24 hours a day, 7 days a week for young adults ages 18 to 27 who are facing homelessness or housing instability in San Francisco.
It is uniquely co-located at the Lower Polk TAY Navigation Center and is considered a one-stop offering a continuum of care at one centralized location. Young adults will have access to services ranging from basic street respite, one-on-one support services, to structured group activities and classes.
All services are designed to uplift and empower young people to build lasting stability and independence.
